Here are a few things you should bring with you to your appointment:

• Your insurance card. We will make a copy of it for your patient chart.
• Co-payment. Most insurance plans require co-payment at the time of service. Our office accepts cash, checks, VISA and MasterCard. Please note that we are happy to assist you with payment terms.
• Any changes in your address or telephone number.

We accept most major insurance plans. if we are not listed among your plan’s participating physicians, please call our office to confirm that your healthcare coverage includes us.
